Graves the sources name
Mirrors of Life are listed at the Cloister, Pauper's Grave, the Ancient Sewers, and the Overgrown Crypt. Sub-areas are the Cemetery Entrance, Cloister, Sepulcher of the Knight, Pillaged Tombs, Serpents' Lair, Ossuary of the Innocents, and the Tomb of the Noble Queen.
Medusa is in the Ossuary. The Trembling Eye is in the Pillaged Tombs, past a trap room, in a mass of flesh. The Sage's arcana is the last stop, in the Tomb of the Noble Queen, after the Hall of Mirrors and the Temptress.
Green tombs
A Spanish search asks about destroying green tombs. Nothing in the English sources describes that as a mechanic. Until it shows up in a chapter or a map legend, it is not a step on this page.
What I ran into
The cemetery looks finished once Medusa drops. It is not. I came back for the Trembling Eye and again for the Noble Queen tomb, which is why those two are in the first screen and not buried under the boss.
